Alternatives to Steroids: Which Ones Make Sense?

Alternatives to Steroids: Which Ones Make Sense?

Steroid alternatives may sound like a way to achieve faster muscle growth without health risks. However, this term covers a wide range of options: from common sports supplements to substances that interfere with hormonal processes. Let’s take a look at what the evidence says about them, what they can realistically achieve, and why the term ‘alternative’ does not automatically mean a safer version of steroids.

Why do people look for alternatives to steroids?

People usually seek faster muscle growth and greater strength, but at the same time they are concerned about the health consequences. On the internet, however, they find protein and creatine listed alongside SARMs, prohormones and peptides. The same label does not make them similar products – they differ in their effects, the quality of the evidence and the risks involved. The term‘steroid alternative’can therefore refer to very different things.

Anabolic-androgenic steroids are substances related to testosterone. They act on androgen receptors and can accelerate the growth of muscle mass and strength, whilst at the same time interfering with hormonal regulation. When misused, reviews describe, for example, increased blood pressure, changes in cholesterol levels and elevated liver enzymes, as stated in a systematic review and meta-analysis of the adverse effects of anabolic steroids. The specific course of events varies depending on the substance, the duration and method of use, and the individual’s state of health.

It is therefore worth first clarifying what this ‘replacement’ is actually intended to replace. If the aim is to support training, some common supplements may help. If someone is expecting a rapid hormonal effect without risk, the available evidence does not support such a claim. It is also important to distinguish between treatment prescribed by a doctor for a specific diagnosis and the use of hormonally active substances to alter one’s physique. The same name does not necessarily imply the same objective, supervision or balance of benefits and risks.

What is considered steroid replacement therapy?

The term ‘muscle-building steroids’ usually refers to anabolic-androgenic steroids. Corticosteroids, which doctors prescribe for conditions such as inflammatory diseases, are a different group of medicines. The term ‘steroid substitutes’ on the internet can refer to products with very different effects and risks.

To help you get a quick overview, it is useful to distinguish between three groups:

Group

What they may offer

An important distinction

Common sports supplements

Protein supplements provide protein, whilst creatine can support performance during intensive training.

They do not act as hormone replacements; results also depend on your training and diet.

Herbal extracts and ‘testosterone boosters’

They are marketed to support vitality or hormonal balance.

The specific ingredient and the evidence are key; the effect of a single substance cannot be generalised to the whole mixture

SARMs, prohormones, peptides and growth hormone

Specific substances or their medical applications are being researched.

These are not interchangeable with ordinary supplements, and the ‘research only’ label is no guarantee of safety.

In the case of substances with a direct or indirect effect on hormonal processes, the specific compound must be assessed. When it appears on a list alongside creatine, this may give the impression that they are equally safe or interchangeable options.

Nor do labels such as ‘natural’, ‘legal’ or ‘for research purposes’ in themselves confirm a product’s safety or suitability for human consumption. In the EU, health claims must be clear, accurate and supported by evidence, as the the European Commission. However, a label is not a laboratory analysis, and online sales do not guarantee that the contents match the stated composition. The legal status depends on the specific substance and its use; athletes must also comply with current anti-doping rules.

What alternatives to steroids are there?

Muscle-building supplements mainly include protein and creatine. Caffeine, beta-alanine or carbohydrate drinks can support a specific type of training. Their role is specific, and none of them replaces the hormonal effect of steroids.

Herbal preparations and so-called testosterone boosters form a diverse group. If a supplement helps someone who is deficient in a particular nutrient, this does not mean it will increase testosterone levels in someone with normal levels. With blends, it is also important to check whether the entire product has been tested in the composition in which it is sold. The NIH’s overview of sports supplements points out that evidence regarding a single ingredient cannot automatically be applied to every commercial combination.

For specific ingredients, the conclusions may even be negative: for example, the NIH review statesthat available research does not support the use of Tribulus terrestris to enhance sporting performance. This is a conclusion regarding one plant and a specific aim, not every plant extract.

SARMs are selective androgen receptor modulators. Prohormones can be converted into hormonally active substances within the body; however, the term itself is also used loosely in marketing, so it is important to examine the specific composition. Further details can be found in the articles SARMs: what they are, how they work and what the research says and Prohormones and designer steroids: effects, risks and what the research says.

Peptides and growth hormone constitute another distinct group. Some substances are being studied in relation to specific medical conditions, but the results of such research do not automatically confirm their efficacy or safety in healthy individuals seeking to build muscle. The article Peptides: what they are, how they work and what the scientific research says explores this topic in more detail.

The Steroid Alternatives category is not a scientific term for a single group. Within this category, you will find products with varying compositions and intended uses; the specific ingredients, evidence regarding the finished product and its risks are the decisive factors.

Nutritional supplements as a safer alternative

If you want to manage your training better and gradually build muscle, start with your diet. Protein provides the amino acids needed for muscle recovery and can be sourced from meat, fish, eggs, dairy products, pulses and tofu. Protein powder is a practical option when you’re unable to meet your intake through food alone. A systematic review and meta-analysis of protein supplementation in strength training have shown benefits for muscle mass and strength when combined with training; the outcome also depends on total protein intake. Protein alone will not bring about progress.

Creatine monohydrate is one of the best-researched sports supplements. It helps to replenish energy during short, intense bursts of activity, such as strength training. A meta-analysis by Wang and colleagues found that creatine, when combined with strength training, promoted increases in upper and lower body strength in adults under 50. However, an increase in lean body mass does not automatically mean an equal amount of new muscle, and early weight gain may be partly due to increased water retention in the muscles.

Creatine may be particularly usefulfor muscle growth as it helps to improve training performance. It does not replace testosterone and does not produce the same hormonal profile as anabolic steroids. It may be of lesser benefit during endurance activities.

Caffeine, beta-alanine or carbohydrate drinks may have a role to play depending on the type of training and the individual’s needs. A long list of ingredients on the packaging does not necessarily mean that the supplement will lead to more muscle. When it comes to supplements, what matters is your specific needs, the composition, quality and the evidence. And if you don’t yet have a regular diet or training routine, the exact minute after training isn’t the first thing you need to worry about.

SARMs, prohormones and peptides

These substances are sometimes described as milder or more targeted versions of steroids. Users most often expect them to deliver faster gains in muscle mass and strength without the same risks. However, a selective mechanism does not mean that the effect is limited solely to the muscles or that the substance has no side effects.

In a controlled study of LGD-4033 involving 76 healthy men , the test substance or a placebo was taken for 21 days. The researchers recorded an increase in lean body mass, but also a decrease in total testosterone, sex hormone-binding globulin (SHBG) and HDL cholesterol. Lean body mass is not the same as pure muscle gain. A short-term study is also unable to confirm long-term safety or demonstrate what happens when using online products with an uncertain composition.

The FDA warns against bodybuilding products containing steroids and steroid-like substances. Possible side effects of SARMs include serious liver damage; the agency also warns of cardiovascular and hormonal problems. Furthermore, an unlabelled anabolic steroid was detected in one of the products tested. These are warnings issued by the US agency; the legal classification of specific products may vary from country to country.

In the case of prohormones, peptides and growth hormone, both the evidence and the risks depend on the specific substance and its use. For example, small studies of androstenedione, one specific prohormone, according to an NIH review , did not show any greater increase in strength or lean body mass compared with a placebo. This result cannot be generalised to all prohormones; the name of the group alone does not prove efficacy. Furthermore, the clinical use of a particular substance does not automatically imply that it is suitable for a healthy person.

Athletes must also take into account WADA’s list of prohibited substances for 2026, which includes anabolic substances, several SARMs and growth hormone.

What does the scientific evidence say?

When evaluating alternatives to steroids, look at what researchers have actually measured. A change in a laboratory marker, such as hormone levels, does not necessarily mean bigger muscles, greater strength or safety with long-term use. A study on cells or animals does not, in itself, prove efficacy in humans.

The following overview shows why the various options cannot all be judged by the same yardstick:

Option

What the research shows

What this does not imply

Protein

A meta-analysis on strength training has shown the benefits of supplementation for muscle mass and strength in the context of training and total protein intake.

The powder alone cannot replace training or diet.

Creatine

A meta-analysis of adults under 50 links creatine combined with strength training to increases in strength in both the upper and lower body.

This is not a hormonal effect, and results vary from person to person.

Herbal extracts

and ‘testosterone boosters’

The evidence depends on the specific ingredient; the results of a single study cannot be generalised to every blend.

The marketing name does not confirm an increase in testosterone or muscle growth.

SARM LGD-4033

A short, controlled study recorded changes in lean body mass as well as testosterone, SHBG and HDL cholesterol.

A 21-day study does not confirm the long-term safety or quality of online products.

Prohormones, peptides and growth hormone

Research focuses on specific substances and often on specific medical applications; the evidence therefore varies.

The results for a single molecule or diagnosis do not confirm the efficacy and safety of the entire group in healthy people.

Anabolic steroids

A systematic review and meta-analysis describe, amongst other things, changes in blood pressure, cholesterol and liver function markers.

The pooled results do not accurately predict what will happen to a specific individual.

For protein and creatine, we have human reviews and meta-analyses that support specific benefits in strength training. However, this is still not hormone replacement therapy, nor is it a guarantee of the same outcome for everyone. For SARMs, there are studies on specific molecules, but the short follow-up periods and small sample sizes do not address the question of long-term safety. In the case of herbal blends or other hormonally active substances, it cannot be assumed that a single study applies to all products with a similar name.

The difference is also evident in the scope of the research: the meta-analysis on protein included 49 studies and 1,863 participants, whilst the controlled study of LGD-4033 followed 76 men over 21 days. The results of these studies cannot be directly compared, but the duration and sample size of the studies help to explain why a short-term trial of a single molecule does not confirm the long-term effect of the entire group.

When reading the study, note:

  • who took part and how long it lasted,

  • what the researchers measured and whether the participants had a control group,

  • whether they investigated a specific ingredient or the finished product,

  • whether the results can be applied to a healthy person looking to build muscle.

Finally, distinguish between evidence for an individual ingredient and evidence for the finished product. Research into creatine monohydrate does not automatically confirm the effect of another form or a mixture with other substances. The promise of ‘synergy’ is, without testing the entire mixture, a hypothesis, not the result of a clinical study.

Is it possible to achieve similar results without steroids?

Natural progress is possible, but its pace depends on training experience, programme, diet, sleep, genetics and time. Before trying another product, it’s worth checking the basics:

  • Training: regular strength training with a gradual increase in intensity.

  • Diet: sufficient energy and protein according to your needs.

  • Recovery: good-quality sleep and time to recover.

  • Supplements: only then should you consider whether protein or creatine addresses a specific need.

Short-term fluctuations in weight do not in themselves indicate how much muscle mass a person has gained. Creatine and protein can support a specific training goal, but they do not mimic the pharmacological effects of steroids. If in doubt, a trainer can adjust your training plan; if you have health concerns or suspect a hormonal problem, it is advisable to speak to a healthcare professional.

Which alternative is the safest?

There is no single substance that delivers the effects of anabolic steroids whilst also being guaranteed to be safe. For most people, the most sensible approach is to start with training, diet and recovery. Protein supplements can make up for any protein deficiency, and creatine can support performance during high-intensity exercise; however, neither is a hormonal substitute.

When choosing a specific product, ask yourself three questions:

  • What results have human studies shown?

  • How long were potential risks monitored?

  • Did they examine the exact ingredient or blend you’re considering?

With substances that interfere with the endocrine system, there is often greater uncertainty regarding both the long-term effects and the composition of a specific product. Be wary of claims of ‘steroid-like’ effects without side effects. If you’re taking medication, have a medical condition or are experiencing unusual symptoms, it’s wise to consult a specialist. Track your progress under consistent conditions, not just by a single morning reading on the scales.

Conclusion

A ‘steroid substitute’ can refer to either a common sports supplement or a substance that interferes with hormonal processes. These options are therefore not interchangeable. The best-supported path to natural progress remains training, nutrition and recovery; protein or creatine may, depending on the situation, support a specific training goal, but they cannot replicate the effects of steroids. With other substances, the specific composition, evidence and risks must be assessed.

Sometimes the best safe alternative to steroids is less dramatic than the label promises: regular training, enough food and good-quality sleep. They do not have their own marketing campaign, but they form the solid foundation of progress.
